I live in the U.S and that's incorrect at least with my provider. We just show up during their hours.

when I lived in the US as an au pair I went to Walgreens to get my flu shot since I didn't have a doctor there

you only get the letter if you are eligible for the free shot, it's not for everyone. if you want one privately you also make an appointment
