sometimes a grave will collapse and the cemetery will have to pile more dirt on top for safety reasons and over time it will settle.

We had to get permission and sign papers for my uncles ashes to be buried above my grandfather. So how this happened is beyond me.

In Michigan, the caskets are actually buried behind the headstone. If this is the case where you are too, it could be someone from the next row.
