🖥️⚡🫀 In this video we see an EKG tracing from a patient post-syncopal episode, now awake and alert. They have continued symptoms indicating hemodynamic instability, and their ECG reveals sustained unstable ventricular tachycardia with a pulse. Synchronized cardioversion is successfully performed, converting the pt to a sinus rhythm with polymorphic ectopy (premature ventricular contractions).  Ventricular Tachycardia with a pulse is unstable when it is accompanied by signs and symptoms of hemodynamic compromise such as: hypotension, acutely altered mental status, signs of shock, ischemic chest discomfort, acute heart failure, etc. In these cases, immediate synchronized cardioversion is called for. If appropriate, and able to be administered without delaying the shock, sedation and analgesia should be provided prior to synchronized cardioversion. 🎉 Bonus Question: What sedation and / or analgesic medications and doses are appropriate prior to cardioversion?
