A new preprint, the GLEAM study, set out to answer one deceptively simple question about endometriosis and GLP1 medications: when people report their symptoms easing on these drugs, is that just the weight loss talking, or is something else going on? GLEAM was a pre-registered international survey of 227 people with endometriosis or adenomyosis who had used a GLP1. Everyone rated how their symptoms changed, and the researchers lined those ratings up against how much weight each person had actually lost. The whole analysis was planned in advance specifically to pull those two things apart. 📈 What they found Overall, 63% met the study’s strict definition of improvement. Then came the interesting part. When you break it down by how much weight people lost, the pattern does not climb the way you would expect if weight loss were doing the work: Gain or under 2% lost: 52% improved 2 to 5% lost: 84% improved 5 to 10% lost: 69% improved More than 10% lost: 58% improved More weight lost did not mean more relief. When the researchers modeled it properly, the odds of improvement barely shifted with each percentage point of weight lost. The weight-loss term sat almost exactly on the no-effect line, and a formal trend test across the groups came back non-significant. So does this mean GLP1s fight endo directly? This is where we have to be careful, because it is tempting to leap straight to “it’s anti-inflammatory, it works on the tissue itself.” The honest answer is that GLEAM is consistent with a weight-independent effect, but it does not prove one. A flat weight-loss result looks exactly the same whether the drug is doing something beyond weight loss, or whether what we are seeing is the placebo and expectation effect, natural symptom flares settling down, or who chose to fill in the survey. The authors are clear that their data cannot tell these apart, and that a mechanism like direct anti-inflammatory action is a hypothesis worth testing, not something they have shown. One detail in their own model underlines the caution: people who started at a lower body weight tended to report more relief, which complicates any clean “nothing to do with weight” story. So the right way to hold this: the signal did not track weight loss, and that is genuinely worth noticing. But this is a patient-led survey preprint, not yet peer-reviewed, and it reports associations, not cause and effect. GLP1s are not approved for endometriosis, and nothing here is a treatment recommendation. The reproductive side matters especially in this community: current guidance is to stop these medications around two months before trying to conceive, and their effect on implantation is still uncertain. If any of this feels relevant to you, that is a conversation for your own doctor, not a cue to start or stop anything. Why it still matters: GLEAM takes something that was only ever patient anecdote and turns it into a specific, testable question. That is exactly what gets a proper controlled trial off the ground. And this community has waited a long time for its anecdotes to be taken seriously enough to test. References: Bas Baudoin, Lincey van Bakel.
