Sometimes the person making the biggest decision is the person who knows the least about the  everyday life. After 17 years in caregiving, I have watched versions of this conversation happen again and again. A crisis happens. Everybody gathers. And suddenly, the loudest voice in the family becomes the decision-maker. Sometimes that person lives out of town. Sometimes they visit only occasionally. Sometimes they haven't been there for the ordinary Tuesdays—the meals, the appointments, the confusion, the good days, the difficult days. But now a decision has to be made. Where should Mom live? Who should care for Dad? Should they stay home? Would assisted living be appropriate? What matters most to them? And here is the part I keep thinking about: Sometimes decisions are being made for people because the conversation was never had with them before the crisis. This isn't about preparing to die. It's about preparing to live. As we get older, there are conversations worth having while we can still clearly say: “This is what I want.” “This is what I don't want.” “This is where I would feel comfortable.” “These are the people I trust to speak for me.” Because Mom shouldn't be introduced to her future for the first time from the passenger seat of somebody else's car. Caregivers and families, I want to hear from you: Has your family ever asked the person receiving the care what THEY actually wanted before making the decision?
