A nurse prevented me from harming a patient on my first day of residency by doing exactly this - Will never forget it. All questions welcome!

i’ve had this situation before, and i clarified with pharmacy and they replete under 4.0, if they’re expected to drop or are on loop diuretics.

As an MD, I love this. It’s about the patient. Not my ego. Neither of us want them harmed. I’m thankful for the second set of eyes. 🥰

As a pharmacist this is what I do ALL day. Tip toe around feelings and phrase questions in ways not to hurt egos. Sometimes it’s to be nice and not

You don’t need to tiptoe. All you say is why are we giving potassium for a K of 3.8?

Or maybe it's in normal range because the supplemental potassium is working. A pt w/ascites is definitely on diuretics. But agree on the way to ask.

if they have any cardiac hx, we replace to keep K at least 4 and Mag at least 2. other than that or lasix, we wouldn't treat 3.8
