i’ve had this situation before, and i clarified with pharmacy and they replete under 4.0, if they’re expected to drop or are on loop diuretics.

Or maybe it's in normal range because the supplemental potassium is working. A pt w/ascites is definitely on diuretics. But agree on the way to ask.

if they have any cardiac hx, we replace to keep K at least 4 and Mag at least 2. other than that or lasix, we wouldn't treat 3.8
