I dealt with something similar last night. Aftermarket wheels without enough threads between the lug nut and stud caused the wheel fall off.

this is local for me. this guy did the mods himself. it's a 2500 so doesn't have the bad vacuum pump issue. it also failed safety inspection 4 times..
