When I found this stray cat, she could barely use her back legs. I brought her to the vet as soon as I could, hoping we'd finally get some answers. Her blood work showed that her body is dealing with ongoing inflammation. She also has anemia. Her hemoglobin and hematocrit are both lower than normal, which means her blood isn't carrying enough oxygen throughout her body. That lack of oxygen can leave her feeling weak and may also affect her muscles. Right now, her hematocrit is 22.3%. If it ever drops below 18%, she may need a blood transfusion. The vet also said she's mildly dehydrated. Her chemistry results suggest that her immune system has been working hard for a long time. Her globulin is elevated, and her albumin-to-globulin ratio is much lower than normal, which is often seen in cats living with chronic kidney disease. Her AST is only slightly elevated, but because her CK is almost three times higher than normal, the vet believes the increase is mainly coming from damaged muscles rather than her liver. Her amylase is also a little high, likely because her kidneys aren't clearing it as efficiently and because her body has been under constant stress. One piece of encouraging news is that her routine kidney values are still within the normal range. That means if this is chronic kidney disease, she's most likely still in Stage 2, where the kidneys are still able to compensate. Her ultrasound also showed long-term structural changes in both kidneys, along with a thickened gallbladder wall that suggests gallbladder inflammation. The vet's current diagnosis is suspected Stage 2 chronic kidney disease. They explained that damaged kidneys produce less EPO, a hormone that helps the body make red blood cells. Over time, that can lead to anemia, which reduces oxygen delivery throughout the body. Without enough oxygen, muscles can become damaged, which may explain her elevated CK level and why her back legs have become so weak. The next step is to complete a urinalysis, a UPC test, and an SDMA test to get a better picture of her kidney health. For the long term, she'll need kidney-support supplements, extra hydration, a prescription kidney diet, and regular follow-up blood tests.
