My Approach to Subcision I always take a meticulous, controlled approach to subcision to ensure maximum scar release while preventing unwanted side effects like sagging. This means working within the correct skin layers: Subcutaneous plane – This is where the fibrotic bands are most commonly found. I navigate through this layer with precision to release the scars without disrupting important facial structures. Avoiding deep fat compartments – Over-aggressive subcision in the wrong layers can disrupt deep fat pads, leading to unnecessary volume loss or sagging over time. This is why expertise in facial anatomy is critical. Collagen stimulation post-subcision – After breaking up the scar tissue, I often pair subcision with collagen-stimulating fillers like Bellafill or Sculptra to restore lost volume and keep the skin lifted.
