@dmd_spa: Case #526: Restoration of severely compromised teeth. Patient presented to the clinic in tears asking for a smile restoration as the state of their teeth has obliterated their confidence. We do sympathize, however we have to be very objective with our aproach.  Extraction of the involved premolar was a reasonable option but there would be a challenge to provide a suitable replacement due to wide spacing. We concluded that restoring the teeth where possible would undoubtedly be a conservative and convenient decision. Upon evaluation, the cavity margin extended below restorable limits under routine circumstances. None-the-less crown lenghtening(gingevectomy/alveolectomy) was done to expose more coronal structure (above gum)  and cavity margin. CROWN LENGHTENING/ROOT CANAL/COMPOSITE CORE BUILD UP/RESTORATION.
