That’s lift OEM would request 8” concrete slab. We supply lifts, we drill & check each floor 1st

I’d Pull the anchors and they make a structural epoxy, inject it and reset new anchors

6 inch minimum, and those look like wedge anchors , should be chemical anchors so washing the floor doesn't corrode the anchors , stronger too

90ft/lbs is the Torque spec I was told to tighten the bolts too. I have the same lift. Good reminder!
