The severity of ADHD symptoms changes across menstrual cycle. 🥼In a study examining daily reproductive hormone levels with daily ADHD symptom expression, declines in E2 predicted clinically significant 2-fold increases in ADHD symptoms of inattention and hyperactivity-impulsivity. Estrogen declines occur at 2 points: 1️⃣Mid-cycle: Just before ovulation, estrogen peaks and then rapidly declines after ovulation. They then rise again slightly. 2️⃣Premenstrually: The week before menstruation, estrogen levels drop again. 🧍🏻‍♀️There are also individual differences. When women had higher average estrogen, they showed fewer attention difficulties. Higher progesterone levels premenstrually seemed to also moderate attention difficulties (reduce them). 🧠Estrogen impacts dopaminergic responses in the brain. A drop in estrogen leads to reduced dopamine activity, worsening ADHD symptoms. In ADHD, dopamine functioning in the prefrontal cortex disrupts executive functions and emotional regulation, while reduced dopamine in mesolimbic regions impairs reward processing. 📉With lower dopamine levels, the brain has a blunted sense of reward. This can lead people to seek more intense or immediate rewards (e.g. risky behaviour) or novelty to find pleasure. It makes it harder to stay engaged with tasks that aren’t immediately rewarding, impairing attention and concentration, and creating a sense of discomfort or tension that can be expressed as anxiety or irritability. PMDD also disproportionately affects those with ADHD, with 46% meeting the criteria compared to 5-8% generally. It’s a severe depressive condition with extreme emotional and psychological symptoms occurring premenstrually - a post for another day! Eng et al.
