Never seen it in 24 years as a Toyota MDT. Looks like a casting defect not caught. Very Rare. 4.0 4Runner

It’s not common. I’ve seen many blown head gaskets on the 1GR, and 2GR. For some reason it’s been cylinder 3 or 5.

pouring something cold in an overheated engine can cause that. know someone that did it. I changed their engine too.

I had a highlander that was overheating and it seemed like it was something like that. Traded it in & coolant steam was billowing out of the exhaust

Put some JB weld on it and sand it down after it dries and you’ll be fine.
