how to stop AI from hallucinating Stop arguing with it. "Be accurate," "don't make things up" — that never works, because hallucination isn't a lying problem, it's a supply problem. Here's the mechanism nobody explains: an AI's default answer is "I can't answer that." What switches it on is recognition — it sees a familiar name, a question shaped like a thousand it's seen, and flips to "I've got this." So it knows the shape of your question, not your facts. Switch on, nothing true to say = a hallucination. Which means there are only two failures: the fact was never in front of it, or a wrong one was. Researchers traced 832 real AI errors — two-thirds broke before the model read a single word. The AI usually isn't inventing things. It was handed the wrong page. The 4 fixes: • Paste the actual document — a better prompt can't fix a missing fact • Give it LESS, not more — Stanford buried the answer in 20 docs and it scored worse than getting nothing. The paragraph, not the folder. • Delete the stale version, don't keep both — and on anything long: "if anything here contradicts anything else, list it before you answer" • "If you don't know, say you don't know" — it's graded like a test-taker, where a confident wrong answer and "I don't know" both score zero, so it guesses. Give it permission to stop. It was never a smarter model. It's what's sitting in front of it when it answers.
