Low back pain is pain felt between the lower ribs and buttocks. Most cases, unless diagnosed specifically, are considered non-specific, meaning symptoms cannot be reliably attributed to one specific muscle, disc, joint, or other structure. Instead, pain is often influenced by a combination of factors including changes in activity or training load, physical capacity, prolonged positions, sleep, stress, recovery, previous pain experiences, and nervous system sensitivity. The sacroiliac (SI) joints connect the sacrum at the base of the spine to the pelvis and help transfer forces between the trunk and legs. SI joint–related pain is commonly felt around one side of the back of the pelvis or buttock. The SI joint naturally moves only a small amount and is supported by strong ligaments and surrounding muscles. While increased laxity can occur with pregnancy or generalized hypermobility, the SI joint is unlikely to simply be “out of place,” and true displacement is rare. For both low back and SI joint–related pain, rehab is often less about “fixing” one structure and more about gradually building your body’s capacity through: • Comfortable movement and mobility • Trunk and hip strength • Rotational and cross-body control • Single-leg strength and stability • Progressive loading • Gradual return to running, lifting, and everyday activities Your spine is designed to move, bend, rotate, and tolerate load. We may temporarily modify movements that are highly provocative, but the long-term goal is to build confidence and capacity. These exercises are examples, not a universal prescription. Exercise selection, range of motion, resistance, volume, and progression should be individualized based on your symptoms, goals, current capacity, and response to loading. Let’s work together!
