This looks like using the wrong oil viscosity This happens when you are using cheap and wrong engine oil

if it an diesel engine then the injectors are not properly seating that produces more carbon towards the piston if not diesel then I have no idea

I believe it been running a little hotter than normal and caused the ring to become weak and started wearing the rings excessively

alot of hard work,ring lands wear to a taper and force the top ring out under compression and break over time
