It’s absolutely reasonable to question all other fastening processes in production line given this incident and finding additional loose bolts.

Every Airline should have a team of technicians inspect new planes to their fleet for all parts and hardware with periodic inspections after that

It was never bolted it. You can tell because there wasn’t any damage around the frame of the door.

When I worked in manufacturing we had “check back marks” checking a previous operators work make sure a bolt is tight. Clearly an operator isssue.
