CNA documented "0/0" for blood pressure a couple hrs before I received the pt for inpatient dialysis. the note said "RN notified" I was like 😒😂😒

I remember a case where an anesthesiologist messed up and didn’t put a guy to sleep and took the surgeon awhile to notice the guy was awake during surgery. They wrote that the surgery went fine
