No matter how stressed they are the RN’s they still have to have 2 nurses to double check. My son just had a surgery and two nurses were always double checking the medication and IV medicine as well.

I had a major near miss when I was qualified less than 6 months. I was 21 hours into a 36 hour shift. Sleep deprivation/fatigue leads to errors.

We have had times where the scanner was down. In that event, I physically look at the order, look at the med, then repeat. Don’t want to hear you don’t have time. You make time.
