Balloon Inflation: This occurs immediately after the closure of the aortic valve, which is identified by the dicrotic notch on the waveform. Upon inflation, there’s a sharp rise in the diastolic pressure. This augmented diastolic peak is higher than the natural diastolic pressure and is the key therapeutic feature of IABP, enhancing coronary artery perfusion. Sustained Inflation: After the sharp rise, the pressure plateaus during the balloon’s full inflation. This plateau represents the period of maximal coronary perfusion. Balloon Deflation: The deflation of the balloon is timed just before the opening of the aortic valve (onset of systole), resulting in a rapid drop in aortic pressure below the patient’s natural diastolic pressure. This sudden decrease creates a vacuum effect that reduces the workload on the heart by decreasing afterload and improves cardiac output.
