The amount of times I’ve gone to a Dr and said “uh hey can you explain why we’re giving/doing XYZ for this pt” and they’re like OH WRONG ORDER WRONG PATIENT. ALWAYS ask if you’re not sure!!

I can’t tell you how many times a physician would order a med, get verified by pharmacist and I caught dosage mistakes, allergies to meds, inappropriate meds. If it looks suspicious Always ask!!
