I have come across that before "on one, sometimes two" pistons, caused by coolant entering comb chamber. It forms hydrogen and even burns injector tip/s "check head for cracks" 💭🤷🏻

Probably a 4.5-7 Cummins. That year used a different piston /injector design that stressed the outer piston with heat.

bad injector caused that running g lean in that cylinder and being ran hard might be tuned incorrectly
