Our understanding of how cholesterol plays a role in cardiovascular disease is not based solely on the fact that cholesterol can be found in atherosclerotic plaque. That was one discovery in a long chain of discoveries that led to our current understanding. The determination that LDL particles, which can carry cholesterol through the blood, cause atherosclerotic CVD was based on over 200 studies, including prospective cohorts, genetic studies, and randomized controlled trials with over 2 million participants and 20 million person-years of follow-up. Those with genetically low levels throughout their lives have very low risk of CVD, and based on RCTs, lowering LDL-C by almost any means results in a predictable reduction in risk. Further, as we’ve refined our understanding with further research, we’ve honed in on apoB, which is like a protein “tag” on LDL and some other lipoproteins being the problem. This is because it acts as a sort of magnet with compounds in the artery wall, which draws it in and initiates the development of plaque. So the higher your apoB, most of which is accounted for by LDL particles, the higher your risk, all else held equal.
