Think inguinal hernia We can’t be sure what this patient is doing to enlarge their lump, but it’s most likely coughing or straining. 🔥 Where are we? ✅ just above the underwear line above the groin Whats our top differentials for an expanding groin lump? ✅ Inguinal hernia ☑️ femoral hernia - but these happen 💡 lateral and below the public tubercle! 🔥 Is it direct or indirect? 💡 Direct hernia - directly protrude though the weakness in the abdo wall (Hesselbachs triangle) and into the Inguinal canal (involves superficial Ring only) 💡 Indirect hernia - squeeze though the deep ring of the inguinal canal and exit via the superficial ring ✅ We’d need a clinical exam to tell.
