@cbcnews: Remember the 2022 Rogers outage that lasted 26 hours and cut 12 million people off from 911 and other network services? The loss of wireless and hard-wired services was caused by human error, according to an independent review commissioned by the Canadian Radio-television and Telecommunications Commission (CRTC). The outage — which impacted mobile, home phone and internet service — was made worse by management and system "deficiencies," according to the review report.
