@emsavenger: Replying to @bearlytogether There actually is a role for Ketamine in cardiac arrest, and that is in CPR-induced consciousness. CPR-induced consciousness is traumatic for the patient AND everyone else around. So sedation is appropriate in this situation. Ketamine is a perfect choice as not only does it accomplish your goals in sedation, but it's a sympathetic adjunct to your efforts. As far as Epinephrine goes, let's not take Epi out behind the barn yet. There may still be a role for it in arrest. I have had 2 patient that have been awake during compressions but went unresponsive when we stopped. I so wish we had a progressive system that would consider this information.

I like the thought process of initial push of 1mg of epi and then infusion of epi 200-300mcg/min. Titrate down rapidly with ROSC. Not much for robust data on it.

My 2nd code as a baby medic I had this situation, patient was in v-fib and compressions were so effective that he would regain consciousness. I had to Ketamine him to keep effective CPR going. It was crazy to see in real life! At least I knew our CPR was effective 😂 he ended up being a neuro-intact save!

Look into the data talking about EPI and post resuscitation quality of life. Yes epinephrine raises ROSK rate slightly but leads to much lower quality of life on average.
