I was told by neurologist I either have dissociative seizures or normal epileptic seizures but waiting on mri to find out which, was told I was unable to do my job properly as I was a support worker

I got her lost letter of proof from the Dr today requesting a home or phone cell assessment as shes housebound I'm sending the copy to pip Do I still have to request it on the form for her 🤔
