This is a method I use when people come into my office in a significant amount of pain that shoots down their leg. Although this method may not work for everyone, it works most of the time for my patients.  Mckenzie extension can work great for a bulging disc in the lower back, to help shift the disc away from the nerve. It is not designed for full herniations. The truth is that if a patient is in this much pain, it will be very difficult to fully evaluate their overall movement because everything will be painful. In this case, I get a lot of subjective information where I simply have the patient explain their symptoms to me without having to do any movement and get started on the repeated press ups as soon as I can. When they are doing the press ups, they should gradually report improvement in either their pain, the distance their symptoms travel, or the range of motion. This would be a greenlight that this is a good method for them. Obviously, if press ups are making their pain worse, and I see no improvement in the range of motion or the distance of their symptoms I would immediately stop. The progression on hands and knees from a core contraction to a bird-dog can help decrease back pain and as I said, I add a side plank on both sides once it’s tolerated which is usually not day one but happens after they’ve been doing the first two exercises for several days or weeks. This brings down the intensity of their pain and allows me to do a better evaluation to see why they had a bulging disc in the first place so we can treat the underlying issue and return them back to normal function.
