I also recommend including a section for employment documentation. accommodations, leaves of absences, tardiness, etc. especially terminations. this can help to build a case for disability.

i would honestly love this but like … hardly any symptoms i have have been truly diagnosed and they just fight me because “i have no proof” but they also wont diagnose me so i “can have proof” 🥴
