LANCER has a mech that lets you activate an unstable state causing you detonate instantly for massive dmg when you would normally be destroyed. It kills you instantly and there's no way to avoid it.

In Blades in the Dark there's a mechanic called flashback where you can make a scene that ends up helping you in your current situation. Depending on how plausible it is determines how much stress.

Infinite Revolution has something similar with Burnouts. If you're going to die, you can activate a special mode of your mech where you become incredibly overpowered for the rest of combat

Maybe not coolest, but I adore the way stress works in Alien. It adds to the dice pool, but can have serious consequences if you roll poorly with it, even going catatonic

in Warhammer 40k Dark Heresy II (maybe others, idk) you can chose to burn your 1 Fate Points to do sth that would be otherwise not possible in the moment. it leads to crazy life saving choices
