@jonjon.jpeg: Using Active errors make it easy to push of responsibility. Engaging with, and solving for, Latent errors is how you save lives. A woman wasn’t just killed, but a teenager (driver) is traumatized, and a community is missing a person. Brainerd is a death trap. #chattanooga #traffic #carculture #pedestriandignity #pedestriansafety #urbanism #tndot

Great explanation! I'm studying public health, and this idea of upstream vs. downstream factors relates to every health issue. Looking at underlying causes really changes how you identify solutions.

I'm a planner and I am trying to make things better. It is hard b/c most DOTs are engineer heavy or don't value the planners input. It is a constant battle to try and get these exact points across.

I think it’s true of most things, even personal growth. We often apologize for the active errors we cause but don’t want to deal with the latent errors that make that active error possible. Hmm. Time to go journal.

Yes!!! Access of gastric tube feeding was changed only after a nurse( or nurses) mistakenly infused G- tube formula into the IV access a while back. Before they look almost identical

The classic saying in root cause analysis is that "people are never the root cause." This approach forces the analyst to figure out how to correct the system instead of finding a scapegoat.

I’m a nurse and as soon as you said latent error I thought, “systemic error” aka most med errors 😆 same wavelength

Thanks for this, I’ve been thinking and saying “blame won’t solve a problem” for years and this is a helpful real world context for the idea.

I work on medical device software. We look at errors that can happen due to design or how it can be used incorrectly

This is part of a larger problem when folks don't want to think complexly at all, and only deal with proximal causation

Side note:using active vs passive language to inherently add blame. Ex The woman was killed by(woman blamed) vs The driver killed a woman(blame to car). Impt w/news reporting violence against women

This applies to the design of the U.S. Healthcare system which kills so many people. And we blame CEOs & doctors.

Symptom vs underlying issue/system… tale as old as media, misdirect and distract the public and never take responsibility or improve the country around us 💔
