If the guy performed proper lockout tag out, i.e. disconnected power, he would still have a hand. the washers just bypassed a failed switch or something.

This is the point of LOTO. If they didn't do it that's 100% users fault regardless of whatever other safety features there are.

ALWAYS unplug heavy machinery & check for capacitors/electric storing devices before cleaning! Remove main panel & look for the schematic on how many there are. Test the voltage. be alert, be safe.

follow proper LOTO and energy isolation and it won't turn on despite bypasses. it was bad practice all around. machine should've been inspected when set up as well.

OSHA requires it to be locked out. Never rely on interlocks alone.

My work is way to lax on operators removing safety mechanisms to the point where an operator ignored the safety sign and turned on the machine while maitnence was in it

I find it wild that not only do people bypass safety but so many don't know about emergency stop buttons. I don't know if this machine has one but I've seen first hand people freaking out and no one..

better question is why are the interlocks basic proxy sensors, should be a safety device with a unique key or pin to interlock, also why wasn't it locked out at the main power disconnect?
