Shoulder Pain? What the Empty Can Test Reveals About Your Rotator Cuff . The Jobe test, also known as the empty can test. Here is how it works. We ask patients to raise their arms to about 90 degrees out to the side, then bring forward roughly 30 degrees into the plane of the shoulder blade. Rotat the arm inward so your thumbs point toward the floor, as if pouring out a can. We then apply gentle downward pressure while you resist. . What are we looking for? Pain, weakness, or both. The maneuver is designed to load the supraspinatus, one of the four rotator cuff muscles that stabilize the shoulder and help lift the arm. When a patient cannot hold against resistance, or the position reproduces their pain, it points me toward possible rotator cuff irritation, tendinopathy, or a tear. Here's the honest part. A positive Jobe test does not confirm a diagnosis on its own. The supraspinatus rarely fires in isolation, so this position also recruits surrounding muscles - it's one piece of a larger picture.
