Laryngospasm notch!: Laryngospasm is a rare complication of procedural sedation, occurring in about 0.3% of ketamine sedations. The vocal cords spasm to a closed position. You’ll recognize it by stridor, which is the sound of upper airway obstruction. In severe cases, there may be lack of air movement and paradoxical abdominal breathing, where the chest drops and abdomen expands as the diaphragm tries to pull air in through a closed glottis. You can hear simulated stridor in this mannequin as the laryngospasm occurs. Let’s take a stepwise approach to management, starting with pinpoint pressure at the “laryngospasm notch”. It’s also called Larson’s point for the anesthesiologist who described it. Push at the base of the skull, between the mandibular condyle and the mastoid process, just inferior to the ear lobe. You can do this while simultaneously performing a jaw thrust. This may release the laryngospasm. It’s unclear how it works: it may be related to the painful stimulus, nerve stimulation, or just the jaw thrust. Step 2 is to provide positive pressure ventilation with a bag. Step 3 is to administer a paralytic like succinylcholine to relax the spasm. It’s a good idea to get the medication ready at the start of laryngospasm so you have it in case other maneuvers don’t work.
