Does this specifically mention parent (mom/dad) in the book or is it open ended enough to be used by an alternative caregiver (grandparent, aunt/uncle, guardian etc).

Love this! I made a similar notebook as a special education teacher. A lot of my boy students receiving services for autism & emotional/behavioral disabilities loved mine & I had great bonds w them!

I have 2 boys. We have a great relationship, you know what I do? I talk to them, spend time with them, enjoy life with them, support them when ever they have a struggle w school or socially.
