I used to work for United healthcare in the appeals & grievances dept as a clinical pharmacist & the reason why it's so diff to get a p2p is bc the MD teams don't want to do them.

I witnessed a surgeon argue with an insurance company who decided to randomly take back their cancer removal surgery approval the morning of for no reason at all. was approved after that call though!
