SRP 1-3 in those two quads Stage 3 Grade A - depending on health history and dental history - I was taught to always to go with the worst tooth

if there’s no hx of srp and no active PDs requiring srp then personally i would perform a prophy and classify them as health on reduced periodontum &refer to perio to eval recession and furcations

this is why I really dont like staging & grading. This sounds like factors other than bacteria with traumatic bite. NSPT isnt appropriate. Def perio referral, root cause needs 2 be determin/ addressed

Periodontitis Stage III - grade B. Grading also dep on med hx (smoking, age, A1C, and others det). Perio maint due bone loss/reduced periodontum. In an official dx you default to most severity.

4346 is for when there is no bone loss. SRP is not needed, no pocket depth indicating it’s needed so it’s a prophy. What’s their risk factors? Maybe 2A

If there’s less than 10% bleeding they’re stable perio, have they had SRP before? If there’s bone loss you can’t do 4346. If they’re class 2-3 furcations theyre automatically stage 3. Same w IP CAL 5

canadian here - would grade localized periodontitis stage II grade b… we would just do a regular hygiene appointment??? we don’t have terminology like SRP, 4346 🙃🙃🙃

I would say 1B because the recession can be due to hard pressure with brushing alone. Is BOP spontaneous? Tissue description? I need to see the mouth and X-rays lol

1. Local stage 1 where 4mm pockets are involved (if interdental). Buccal/lingual recession only counts towards perio diagnosis if the pockets are over 3mm

Stage I w/ loc stage III grade B. If it’s loc recession with furcation, wouldn’t it be recession causing bone loss and not necessarily perio? If so, I would keep it at prophy and offer perio referral.

Stage III we need thier age to grade them. You go by the two worst sites to stage. I would ask have they had an SRP in the past. If they have I would do a maintenance and re-evaluate in 3 mos.

This…this is why I only want to see children now.. I have been a hygienist 23 years and have literally performed thousands of quads of SRP but have become highly annoyed at staging and grading 🤦🏼♀️

If patient has a history of SRD, I would do a 4910 and monitor.
