I can only surmise that the blackhawk visually mistook the heading of American Eagle. The plane was on point, in line with 33 at 400ft. What seems like a normal path has a last minute left turn. The blackhawk was heading straight down river and ultimately straight into the oncoming planes path.

ATC did notice and told the blackhawk to move. blackhawk confirmed they had visual but did nothing. 100% fault of the pilot and no one else.

The plane you see at the top is supposedly the one the Blackhawk pilot thought the control tower was on about, and also if you are flying low in a heli it easy to get confused with lights
