Also sometimes just bumping the cervix with the speculum can cause some bleeding. It is very delicate.

Some of my clinics still use the spatula and brush. Still no biopsies or pieces removed. And it's not usually uncomfortable at all. Imagine a kitchen spatula - just miniature.
