That’s strange. First responders are the police in nearly every OD case. The PD is first on scene and have Narcan with them to help you.

I once had a seizure and my wife called 911. A cop rushed into my house (not the paramedics) and gave me narcan thinking I was having an overdose. Pretty sure I’m supposed to be dead from that lmao

the few people I've known who reversed on narcan were either in extreme pain or swinging on paramedics. like 5 days of withdrawal at once is not pleasant

in my state this comes from a culture of if you told the ambulance the reason they need to come is someone overdosing they would wait hours to go do it even if it was a ten minute drive
