Lowkey panicked. This could be cardiogenic shock with elevated BNP and hypotension in the setting of ESRD or acute on chronic kidney disease. I’d go for levo with 500 ml NS over an hour

So question would be is the hypotension the result of the heart failure or is the hf secondary to another problem, say sepsis. If it’s HF levo/epi and maybe escalate to mechanical assistance.
