Rehab for Insertional Achilles Tendinopathy vs Mid-Portion Achilles Tendinopathy looks different - here’s what you need to know 👇 Insertional tendinopathy is characterized by discomfort of the Achilles tendon where it inserts on the heel bone. Understanding this allows us to tailor rehab better to fit the needs of this specific type of condition. One thing we know about Insertional Achilles Tendinopathy is that it does not like compression. How do we cause compression to soft tissue? By stretching a muscle or tendon to its end range. To decrease compression to the Achilles tendon, early rehab should involve working through ranges that allow for minimal compression but still load the tissue. This means performing an exercise at mid-range ankle dorsiflexion or one that favors more plantarflexion to take stress off the Achilles tendon, this can be dependent on the severity of symptoms.   Try the above exercise with your heels resting on a step if you are dealing with Insertional Achilles Tendinopathy! This is a fantastic entry point to load the tendon while working through a non-compressive range of motion. For Mid-Portion Achilles Tendinopathy, we typically experience discomfort in the middle section of the Achilles tendon. With this specific diagnosis, we can generally tolerate working at those end ranges of motion as compressive forces do not typically reproduce symptoms.  For both, the introduction of rehab and more importantly, rehab with LOAD early is paramount to success.
