At most stores they ask that because there are restricted access product like cigarettes, chewing tobacco, lottery tickets and alcohol depending on the size of the store.

I think they're usually just asking if you want any things usually grabbed after the transaction (ice, charcoal, firewood, propane, etc) or things behind the counter (cigarettes, lotto tix)
