No problem!…sometimes… Patients on ECMO, Impella, or LVAD can be perfusing without a palpable pulse because these devices provide continuous flow—eliminating the normal pulsatile pressure wave in a sick heart. Increased afterload can dampen pulsatility, especially in mechanical support device patients, where high systemic vascular resistance (SVR) can reduce native heart contribution.
