Ok just had a cast for a few weeks on my girl and by far the best for cushion and also absorbance (hello diaper leakage) are Poise pads!! They come in different strengths (equal to level of cushion)

It should have a rubber cap on it, someone didn't put it on.If they didn't give you the tips in the brace box, using a silicone putty or paint on silicone like Flexseal would work.

there's this moldable plastic that you can heat up and cover that metal with so it's soft/bouncy and smooth

Silicone corner/edge protectors (you know the kind that parents use for toddlers to protect from falls), you could pair it with a soft sheepskin fleece to protect his arm. Or you could pair it with vetwrapping his arm where it is rubbing, so that the vetwrap is getting rubbed instead of him.

Can you wrap it with the self adhesive bandage wrap? That should make it softer and stop scratching his arm.

I had a similar cast after hip surgery. Quilt backing. The insulation middle layer of a quilt. It’s soft, airaited to stop a rash and easy to get at any fabric store.

I would do a foam noodle over the metal part. Also we use large cloth diapers instead of the outside diaper you are using as we felt like it held better. So the small tuck diaper than a cloth diaper.

ask a welding or mechanic shop in your area if they can take their electric shaver thing (I dunno what it's called, but it spins and shaves down the sharp edges). my dad was a mechanic and had one when I got my ortho braces and was able to shave down all my sharp edges and hinges
