In May 2007, Kenya Airways Flight 507 (a newly delivered Boeing 737-800) took off from Douala, Cameroon, in the middle of a pitch-black night while navigating around severe thunderstorms. Shortly after liftoff, the extremely domineering and overconfident captain made a critical error: he verbally announced that he was engaging the autopilot, but he never actually pressed the button to activate it. The 737 rolled almost completely upside down and entered a fatal spiral dive, plunging into a dense mangrove swamp just miles from the airport and killing all 114 people on board. The investigation became a primary case study on the dangers of spatial disorientation and a complete breakdown of Crew Resource Management (CRM).
