one pattern I've noticed with the investigation of airplane crashes is that they always blame the pilots. till some private investigators find that the plane had issues prior and that companies responsible are never held accountable. maybe this case is different but i always keep that in mind

Everyone is at fault here. The pilot should’ve listened or asked for help from ATC or the instructor. The instructor should’ve taken control. ATC should’ve been more on top of the sequencing.
