You know after Tomoya makes his wish during or after Ushio’s death, he ends up dying too. When he falls to the ground, that’s him dying from heartbreak. While he was taking care of Ushio, he never took care of himself. So when he lost her, it was just too much—he broke down and died right there.
All the orbs Tomoya got from helping people in the arcs show up in that empty field world. After he forgives his dad and starts raising Ushio, he gets the final orb. Then, when Ushio dies in the real world, he makes the wish. At the same time in that other world with the robot and girl, the robot is trying to save Ushio too.
The snow in that world represents Nagisa’s death. Ushio can’t handle the cold in both worlds—same way she gets sick during winter in the main timeline.
As both Tomoya and the robot are losing Ushio, Tomoya makes the wish. Right after he collapses, the screen goes white. Then in the robot world, there’s a flash—Ushio says “Thank you for everything.” The robot explodes—that’s when the wish is granted.
Then we see Tomoya being given a choice—meet Nagisa or not. He realizes meeting her was the one thing that made his life better. He accepts that truth and everything he went through. Because of that, he’s rewarded: the illness never happens, Nagisa lives, and they all get the happy life they deserved together.
