Assessment:
Likely Pseudomonas aeruginosa infection of the nail (Green Nail Syndrome), secondary to prolonged moisture exposure and possible nail plate separation.
Plan:
• Recommend keeping the nail dry
• Start Ciprofloxacin 0.3% ophthalmic drops or Gentamicin 0.3% drops, apply to affected nail BID
• Initiate daily vinegar soaks (1 part white vinegar to 3 parts water) for 10 minutes
• Debride or trim detached nail portions as needed
• Avoid nail polish and artificial nails during treatment
• Follow-up in 2–3 weeks
• Consider oral fluoroquinolones (e.g., ciprofloxacin) if topical treatment fails or infection worsens

fungus.. needs to be placed on antifungal meds.. and maybe a broad spectrum antibiotics too.. topical also. avoid any nail application for 6 months to 8 months.. or ever..
