One of the more frustrating things I see in MCAS is how difficult it can be to actually correct nutrient deficiencies. Someone has low iron. They take iron religiously. We recheck it months later…and it’s still low. And it’s not just iron. We see this with copper, zinc, vitamin C, folate, and other nutrients. Part of this is straightforward: many people with MCAS have significantly restricted diets because of food reactions, so inadequate intake can absolutely contribute. But I also see people with persistent deficiencies despite seemingly adequate intake and supplementation. We can’t prove that MCAS itself is causing malabsorption, but I suspect that in some patients, ongoing GI inflammation and mast-cell activity may affect how nutrients are absorbed or handled. That makes repletion much harder. Sometimes we have to change the form, route, or amount of supplementation—but I’ve also seen deficiencies become easier to correct as we get the underlying MCAS under better control. Sometimes replacing the nutrient isn’t enough. We also have to ask why it isn’t staying replaced. And, as always, both MCAS treatment and nutrient replacement need to be individualized. #mcas #heds #dysautonomia #mastcellactivationsyndrome #irondeficiency
